How do I get started?
Step 1: Create an account at https://formdr.com/ . There is a $240 discount code for the first year of formdr. Please use: trackstatfd (this lowers business annual tier to $39/mo for a total of $468)
Step 2: Create a packet of forms in your FormDr account. Please choose from one of the following TrackStat templates to get started:
TrackStat Dynamic Simple
TrackStat Dynamic Insurance MVA Wellness Form
TrackStat Dynamic Longer MACRA & MIPS Questions
Step 3: Save link in Account Settings - click on triangle icon in upper right corner of TrackStat, select Account Settings. Scroll down to Form Link field and copy and paste FormDr link. Save changes.
Note: you must have Administrator privileges in TrackStat to make these changes.
Step 4: Map fields - enter FormDr field values in TrackStat
Step 5: Integration - connect your form to TrackStat
*Note - FormDr is not affiliated with TrackStat and does require a separate monthly or annual paid subscription to use their software. The FormDr integration is not available for Eclipse customers currently. Patient's chief complaints from FormDr forms are NOT populated in the Subjective portion of the SOAP note in the EHR/EMR.
