Step 5: How do I connect or integrate FormDr with TrackStat?

After you have created a Packet of forms in FormDr, you must connect your form with your TrackStat account.

Log in to FormDr https://app.formdr.com/login

Click on Integration section

Locate TrackStat integration (scroll down)

Connect to TrackStat

Follow prompts to enter TrackStat username and password

select Authorize

Select form from dropdown

Select clinic from dropdown

Enter FormDr password

Select Authorize
