What happens when a new patient fills out the paperwork? Unable to access forms due to mismatched names

New patients can be scheduled in house (directly in EHR) or through TrackStat's online scheduling. Here is the flow:

Online Scheduling

If a new patient schedules through TrackStat online scheduling then a new patient and appointment are created in the EHR. The patient is prompted to complete the FormDr forms. When the forms are complete a notification will be received in the TrackStat Inbox. The demographic data from the FormDr forms will be pushed into the EHR (data available varies by EHR).

Patient scheduled directly in EHR

If a new patient is scheduled directly in the EHR, the patient can be sent an appointment confirmation (text and/or email) prompting them to complete the FormDr forms. Forms can then be merged with the patient in the TrackStat Inbox. The demographic data from the FormDr forms will be pushed into the EHR (data available varies by EHR).

Patient demographic data is automatically linked to the patient if the name and another unique identifier, such as email or phone number, matches

Patient demographic data is NOT automatically linked if the form submissions name does not match the EHR information. However, you can manually merge it from the inbox.
