Integrations - Forms - FormDr - The patient's demographic data is not populating in my EHR/EMR from their intake forms. What do I do?

Why isn't the patient's address from their form importing or pushing into the EHR/EMR?

There are a few things that could be happening?

  1. FormDr is not connected to TrackStat

  2. Your FormDr link is not in the form link box in the TrackStat > Account Settings

  3. Your fields are not mapped.

Any of the above will lead to the fields not importing into your EHR.

    1. TrackStat > Account Settings section

      1. Get the form link from app.formdr.com

      2. Paste this in the form link merge field in TrackStat > Account Settings

    2. Field mapping, currently is in Extras (3 dots) > Integrations > FormDr

  • Why doesn't my drop down option NOT map properly?

      • 2=field value

      • 3 and 4. If you have a question with drop downs, the drop downs need to match the display name. We are not looking for option01. We will search for 'Single' or 'single'. The easiest way to fix this with the Trackstat template, which is set up to map.
