Connect formdr in to TrackStat
Go to app.formdr.com integrations section.
Connect specific form to Trackstat. If you get an error, you will have to contact support about an upgrade. If it is not connected, forms will be sent, but data from the form will not be imported into your EHR/EMR. You will not receive a notification when the form is complete, along with other benefits.
Paste the specific form link into the 'form link' box within the integrations> formdr> area or the account settings.
Get the link from app.formdr.com, then go to trackstat and paste the link into the form link merge field
To turn off FormDr alerts about completed forms, disable the Form Builder Alerts setting.

How do I stop messages from being sent from FormDr?
Go to My Account > turn off Form Builder Alerts
Why doesn't my field with a drop down map? If you started with the template this would not be an issue, but if you created your own question, here is what the issue is. Below you will see a question has a display name, field value which is what we refer to and then we look at the field value of the drop downs. If your drop down is option01, option02, we are not searching for that, we are searching for words that match the EHR field options.

1=field display name
2=field value
3 and 4. If you have a question with drop downs, the drop downs need to match the display name. We are not looking for option01. We will search for 'Single' or 'single'. The easiest way to fix this with the Trackstat template, which is set up to map.
