Intake forms

Introduction

This guide explains how to integrate IntakeQ with TrackStat using API access and webhooks. Once configured, patient demographic information submitted through IntakeQ forms can automatically sync and update in TrackStat.


Step 1: Enable API Integration in IntakeQ

  1. Log in to your IntakeQ account.

  2. Go to Settings.

  3. Scroll to the Integrations section.

  4. Under API Integration, click Settings.

  5. Enable API Access.

  6. Display and Copy your API Key.

Step 2: Connect IntakeQ to TrackStat

  1. In TrackStat, go to:
    Extras (three dots) → Integrations → IntakeQ

  2. Click +Add Integration

  3. Paste the IntakeQ API Key into the API Key field.

  4. Enter a name for the integration.

  5. Save the integration.

Step 3: Add the Webhook in IntakeQ

  1. Copy the webhook URL below and replace PracticeID with your actual TrackStat Practice ID:

2. To find your Practice ID in TrackStat:

  • Click the triangle icon in the upper-right corner.

  • Select Account Settings

  • Copy the Practice ID

3. Paste the updated webhook URL into the webhook field in IntakeQ.

Important: IntakeQ may not automatically save webhook changes. Refresh the page afterward to confirm the webhook URL was saved successfully.

Step 4: Save the IntakeQ Form Link in TrackStat

  1. In TrackStat, click the triangle icon in the upper-right corner.

  2. Select Account Settings.

  3. Scroll down to the Form Link section.

  4. Paste your IntakeQ form link into the available field.

  5. Click Save in the upper-right corner.

Important: Changes will not apply unless you click Save.

Note: Administrator privileges are required to update Account Settings.

Step 5: Find Your IntakeQ Form Link or Embed Code

You can either:

  • Share a direct IntakeQ form link with patients, or

  • Embed the form directly on your website using an iframe.

To Find the Form Link or iframe Code

  1. Log in to IntakeQ.

  2. Click My Forms.

  3. Select the form you want to use.

  4. Click the 3 dots at the top of the page.

  5. Select Get URL.

Available Options

Direct Form Link

Copy the standard URL and paste it into TrackStat or share it directly with patients.

Website Embed (iframe)

Scroll down to the HTML version section and copy the iframe code to add the form to your website.

Step 6: Merging Intake Forms

  • Intake forms with a matching name and phone or email field will automatically merge.

  • Forms that do not automatically match can still be merged manually.
